Driven to Improve Lives: Jefferson's 2025 Community Impact Report

In 2025, Jefferson continued to lead as an anchor institution deeply rooted in the communities we serve and advanced health, opportunity and economic mobility across the region. Our colleagues worked across the Lehigh Valley, Greater Philadelphia and South Jersey to strengthen communities through intentional investment, collaboration and community‑driven solutions. 2024 report is available here as well.

Driven to Improve Lives. Wherever is Needed.

At Jefferson, care and learning don't stop at our doors. We're bringing it into neighborhoods - through mobile health services, community partnerships and programs that expand access to real resources and support. Because improving lives means showing up where it matters most.

The Six Pillars of Healthy Communities

Access to Healthcare

Improving accessible, patient-centered care through partnerships and programs

Through education, health services, and wellness resources, we remain committed to meeting patients where they are, breaking down barriers, and promoting equity in healthcare access.

Access to Healthcare
Community Engagement

Creating meaningful change through partnership, volunteerism, and direct involvement

Sustainable change happens through our partnerships, volunteerism, and direct involvement.

Community Engagement
Healthy Food Access

Providing nutritious food that fuels community health and well-being.

Food insecurity contributes to chronic health issues, including diabetes and heart disease. We mobilize volunteers and partners to tackle this challenge on multiple fronts.

Healthy Food Access
Policy and Advocacy

Shaping policy for sustainable, community-wide health improvements.

Our Community Health Workers Academy joined a coalition of advocates from across PA to secure funding for vital resources, just one of the ways we are advocating for change.

Policy and Advocacy
Safe and Accessible Environment

Building safe, supportive communities.

By investing in trusted community-based partners, we’re expanding access to resources that support health, connection, and quality of life.

Safe and Accessible Environment
Social Support

Investing in the emotional well-being of our neighbors.

We increased our work with mental health and wellness programs, support networks, peer-led community programs, and supportive resources for seniors, caregivers, and vulnerable populations.

Social Support
Building Healthy Communities

As we enter our third century, we look ahead to scaling JeffCARES — our  signature volunteer and community engagement program.
